This pack includes:

• 1 x Sunflower Extra card
  Made from durable plastic. The text on the back of the card is fully editable.

Personalise the back of your card with:

• Your photo
  Upload a clear, high-resolution photograph of yourself.
• Your name
  Add the name you would like people to use when addressing you.
• Details of the support you need
  Choose up to five icons to show the type of help you require.
• An emergency contact
  Add a name and phone number to be contacted with your consent or in case of emergency.

Important: Please double-check your card preview before placing your order. We’re unable to offer refunds for errors or changes after your card has been printed.

A few things to note:

• The views and information displayed on the back of your card are your own and do not represent Hidden Disabilities Sunflower Scheme Ltd or its employees.
• We securely store created cards and their information for three months. After this time, both your card and any related details are permanently deleted as part of our privacy commitment. For more information, please read our Privacy Policy.

Please note: You can order one unique Sunflower Extra card per order. Each card is linked to its own custom Sunflower Extra page, created specifically for that order. However, you can request as many copies of the same card as you'd like.
