Media consent form – Sunflower Extra Films
Summary of what you’re agreeing to
This form gives Hidden Disabilities Sunflower (HDS) and its partners Heathrow, KLM, and Hilton permission to use the Sunflower Extra Films we made together.
- The films may include your name, image, voice, and story.
- They will be used to raise awareness of the Sunflower on social media, websites, events, advertising, and other channels.
- They can be shown in countries where the Sunflower is active.
- Your films may be edited into shorter clips, stills, or combined with other content.
- You will not be paid for this use, but you can choose to stay anonymous if you prefer.
- You can withdraw your permission later by emailing [email protected], though films already published may still be used.

- I give my permission to Hidden Disabilities Sunflower (“HDS”), its partners Heathrow, KLM, and Hilton, and their authorised third parties to use the Sunflower Extra Films produced for this project (referred to here as the “Assets”), including my image, voice, name, story and/or contributions contained in those films.
- I understand the Assets may be published and shared across a wide range of channels, including (without limitation) social media, websites, presentations and events, internal communications and training, printed materials, advertising, press, broadcast, and other media now known or later developed, to promote and raise awareness of HDS, Heathrow, KLM, Hilton, and the Sunflower.
- Territory: I understand this permission applies worldwide, but only in regions where HDS and the Sunflower have an active presence. This permission is perpetual and royalty-free.
- HDS and its partners (Heathrow, KLM, and Hilton) may make reasonable edits or adaptations to the Assets (including clips, still images, or compilations) provided use is lawful and consistent with the purposes above.
- I understand the Assets may identify me by name and may refer to my lived experience/condition to help explain the story. I can choose to remain anonymous in the form below. I will not bring privacy complaints in relation to uses of the Assets within the scope of this consent.
- Ownership: I acknowledge that I do not own copyright or other rights in the Sunflower Extra Films produced by HDS. If I provide any of my own materials for inclusion, I retain ownership but grant HDS, Heathrow, KLM, and Hilton a non-exclusive, transferable, sublicensable, perpetual, royalty-free licence to use those materials as part of the Assets in the regions described above.
- Withdrawal: I may withdraw consent at any time by emailing [email protected]. Withdrawal will stop new or further use of the Assets. I understand previously published or distributed versions may continue to exist, and HDS and its partners may need a reasonable period to remove content from managed channels where feasible.
- I understand I will not receive financial compensation for the use of the Assets unless agreed separately in writing.
- To the extent permitted by law, I waive any moral rights and release HDS, Heathrow, KLM, Hilton, and their authorised third parties from claims arising from lawful use of the Assets as described above.
Who can give consent?
The creator/owner of the Content must give consent. In addition, if the Assets include an identifiable individual (“Model”) or multiple Models, each Model must give consent:
- Individuals aged 18 or over who understand this consent may sign for themselves.
- A parent/legal guardian must sign for anyone under 18 or for a person who cannot sign for themselves.
