Personalize the Sunflower PoTS card by adding your name, photo, an emergency contact and up to 5 icons to describe the type of help you may need.
The card comes with a standard or a small Sunflower lanyard, a rigid plastic wallet to protect it as well as a Sunflower pin badge.

This pack includes:
- A choice of a one standard 85cm or one small 70cms Sunflower lanyard with a quality metal trigger clip
- One Sunflower lupus card in durable plastic
- One rigid (heavy duty) protective plastic ID card holder
- The text on the front of the card is: I have PoTS
- The text on the back of the card is: I may become lightheaded, nauseous, or faint. Please ask me how you can help and guide me to a place to sit or lie down.
- One 30mm circular stainless steel pin badge with back butterfly clip
Personalize the Sunflower PoTS card by adding your details onto the back.
On the back of the cards, you can:
- Add your photo - upload a high-resolution, clear photograph of yourself
- Add your name - add the name that you want people to use when addressing you
- Add details of the type of help you need with icons - use one of our icons described here. You can add up to five.
- Add a contact - add a name and number to be contacted with your consent or in an emergency
