The text on the front of the card is: I have chronic dizziness

The text on the back of the card is: My chronic dizziness means any of the following can be affected: balance, memory and mood. I may also experience vertigo. Please be understanding and ask how you can help.

This pack includes:

  • A choice of x1 standard 85cm or small 70cm lanyard-The lanyard is made from recycled PET material with a degradable plastic safety break
  • x1 ID card in durable plastic
  • x1 Plastic wallet to protect ID card
  • x1 30mm circular stainless steel pin badge with butterfly clip

On the back of the card, you can:

  1. Add your photo - upload a high-resolution, clear photograph of yourself
  2. Add your name - add the name that you want people to use when addressing you
  3. Add details of the type of help you need with icons - use up to 5 of our icons
  4. Add a contact - add a name and number to be contacted with your consent or in an emergency
